Primary Care and Direct Access
At Heartland Care Solutions we care about all aspects of your health and recognize that specialty care is necessary for certain chronic conditions like mental health ailments and chronic pain. As such, we do not prescribe or manage medications to treat these conditions that would be best addressed by a specialized professional. We are happy to assist with coordinating referrals as necessary to treat conditions outside the scope of primary care!
For $2-3/day (less than your daily cup of coffee) Members receive
Direct access to your provider when you need it!
Annual wellness visits
Acute illness and injury care
Chronic disease management
Medication management
Telehealth and direct messaging capability
House calls may be available under certain circumstances
Other Services

Department of Transportation (DOT) Physicals do not require membership and are performed by a Certified DOT Medical Examiner.

Heartland Care Solutions provides acute illness care to non-members at a cash pay price that will be discussed prior to being evaluated. Payment is due up front.
